2011 IEBA Showcase Submission

This form is a request for an act to be considered for a showcase spot at the 2011 IEBA Conference and does not serve as a contract or performance guarantee in any way. Please be advised that final decisions will not be made until September 1, 2011 and you will be notified by IEBA regardless of whether or not the act will be receiving a showcase spot.
 
Please submit one form per artist. Do not resubmit an artist if you have already submitted said artist for 2011 review. 
 
Showcasing cost will be determined after selections have been made and time slots have been confirmed. 

Artist Information

Including band members.

 MySpace, Facebook, YouTube pages are valid.

Artist Media

 Check any media pieces that will be submitted.

Please attach any media samples for review. 20MB file maximum.
Acceptable formats: .gif, .jpg, .eps, .pdf, .txt, .html, .doc, .ppt, .xls, .avi, .mov, .mp3, .wav, .rar, .zip
